Beacon Home Health, an affiliate of The Pennant Group, is seeking a Registered Nurse to join our Home Health team in Knoxville, TN. You will provide and direct nursing care to patients in their homes, coordinating with physicians and other professionals to develop and implement a comprehensive plan of care.

Responsabilidades

• Provide nursing care to patients in their homes as prescribed by the physician and in accordance with laws and agency policies.
• Coordinate the total plan of care with other health care professionals to maintain continuity of patient care.
• Perform initial/comprehensive nursing evaluation visits and develop the plan of care.
• Document patient condition, goals, interventions, and responses to home health care.
• Deliver high-quality clinical services within the scope of practice from admit to discharge.

POSITION OVERVIEW:

The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. A few position highlights include:

• Coordinate the total plan of care with other health care professionals involved in the care and strive to maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
• Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.
• Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.

THE MUST HAVES:

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
