Job summary

Ascension at Home together with Compassus is seeking a Home Health Registered Nurse (RN) for Baylor Shift in Evansville, Indiana. Youll support patients at home, promote independence, educate families on disease management, and coordinate care with an interdisciplinary team to enhance quality of life.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse; home health experience preferred.
• Current drivers license.
• Current CPR and BCLS certification.

• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
• Educate patients and families on disease management to reach healthcare goals.
• Case manage and provide clinical oversight to interdisciplinary team.
