Job summary

Rosaires Care is seeking a Registered Nurse to provide compassionate home- and community-based care. You will assess patients, develop and update care plans, and perform nursing interventions including medication administration in clients’ homes.

You will collaborate with physicians, therapists, and families to ensure safe, effective outcomes while promoting safety, independence, and dignity for seniors and individuals with disabilities.

Qualifications

• Current RN license and strong clinical nursing skills.

Registered Nurse – Home & Community-Based Care

Provide compassionate, client-centered skilled nursing in clients’ homes. Perform health assessments, develop and regularly update care plans, and deliver nursing interventions including medication administration and prescribed treatments. Coordinate care with physicians, therapists, and family/caregivers to support safe, effective outcomes.

Promote client safety, independence, and dignity while delivering individualized care for seniors and individuals with disabilities. Work collaboratively within a multidisciplinary team and maintain required clinical documentation and compliance standards.
