Qualifications

• Active RN license in Illinois.
• Minimum 2 years of RN experience.
• Ability to travel to Oak Forest and surrounding areas.
• Experience in home health care is a plus.

Responsibilities

• Travel to patient homes in Oak Forest, IL and surrounding areas.
• Perform comprehensive assessments of patients' physical, emotional, and social needs.
• Develop and implement individualized care plans to achieve optimal patient outcomes.
