Job summary

Orion Home Health is seeking a Licensed Practical Nurse (LPN) to join our home health team providing patient-centered care in patients' homes across Illinois. You will collaborate with an interdisciplinary team to deliver high-quality care, administer medications, assess conditions, and educate families on self-care and disease management.

Qualifications

About the Role:

• Administering medications and treatments as prescribed by physicians
• Assessing and monitoring patient conditions, reporting changes to healthcare providers
• Educating patients and their families on self-care techniques and disease management
• Collaborating with healthcare professionals to develop and implement individualized care plans
• Maintaining accurate and detailed patient records
• Maintaining a safe and clean environment for patients and staff
