Qualifications

• Current and unrestricted RN license in Florida.

• 1+ year of clinical experience in home health, hospice, or community health.
• Excellent communication and interpersonal skills for working with patients, families, and healthcare providers.

Responsibilities

• Conduct comprehensive patient assessments in the home setting to evaluate physical and emotional health.
• Develop and implement individualized care plans based on physician orders, patient goals, and clinical best practices.
• Administer medications, injections, wound care, and other treatments as prescribed by the physician.

• Coordinate care with other healthcare providers, including physicians and therapists, to ensure continuity of care.
