Mental Health Association of Rochester seeks an experienced Registered Nurse to provide skilled nursing care in patients' homes. You will perform comprehensive assessments, administer treatments, manage wounds and IV therapy, and coordinate with an interdisciplinary team to support recovery and safety.

Responsibilities include updating care plans, documenting in OASIS and EHR systems, and educating patients and families to promote self-management and independence.
