Personal Touch Home Health Care Services Inc. is seeking a Home Health Nurse to provide compassionate, high-quality care to patients in their homes, delivering skilled nursing with a focus on safety, comfort, and independence.

Responsibilities include conducting patient assessments, developing and updating care plans, administering medications and IV therapies, educating patients and families, coordinating with interdisciplinary teams, and documenting progress in EHR systems to ensure continuity

Competenze

• Strong clinical nursing skills suitable for home-based care.
• Experience with home-based care and patient popuations.
• Proficiency with EHR documentation and care planning.
