Job summary

Home & Care Home Health is seeking a full-time Registered Nurse (RN) to join our Casper, WY home health team. You will deliver high-quality nursing care to seniors in their homes, supporting independence and comfort in familiar surroundings.

You will assess needs, educate families, coordinate with an interdisciplinary team, and maintain detailed patient records while adhering to the highest standards of care.

Qualifications

• Wyoming RN license required.
• New graduates welcome to apply.
• Ability to work flexible hours evenings weekends holidays.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to seniors in their homes, assessing physical, emotional, and social needs.
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team.
• Administer medications, treatments, and therapies as prescribed by physicians, and monitor responses.
• Educate patients and families on disease management, self-care, and wellness strategies.
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
• Maintain accurate and detailed records of patient interactions, assessments, and care plans.

Responsibilities:
