Job summary

Qualifications

• Registered Nurse or Physical Therapist with Florida licensure required or in process
• OASIS Certification required; Home Health Coding Certification required (within 12 months)
• 2 years of Case Management experience preferred, including Home Health case management
• Bachelor’s degree preferred for RN or PT along with relevant licensure
• Must have valid Florida driver’s license and be able to commute to Fort Myers

Responsibilities

• Analyze SOC, Recertification and Resumption (and other follow-up OASIS) documentation.
• Ensure proper utilization of resources to achieve positive patient outcomes and optimal financial outcomes.
• Cover team PTO and perform other office tasks as needed.
• Work under the direction of the Director of Home Health Quality and Compliance.
• Maintain professionalism, teamwork, and flexibility in a hybrid-office environment

Location: Home Heath Offices: Westlinks Drive, Fort Myers FL 33913

Department: Quality Improve - Education

Work Type: Full Time (80 hours bi-weekly) Exempt/Salary (Hybrid Schedule)

Shift: Days, 8:00:00AM to 4:30:00PM

Minimum to Midpoint Pay Rate:$30.27 - $40.86 / hour +

Summary:

Office based position under direction of the Director Home Health Quality and Compliance. Responsible for analysis of SOC, Recertification and Resumption (and Other Follow-up OASIS) OASIS and accompanying documentation, management of proper utilization of resources to ensure positive patient outcomes and optimal financial outcomes.

Teamwork, flexibility, and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering team member PTO, and other requested office tasks.

Requirements:

Education:If a Registered Nurse, Associate degree in nursing required, Bachelors degree preferred. If a Physical Therapist, Bachelors or Masters degree required, DPT preferred.

Experience: 2 Years of Case Management preferred. 2 Years of Home Health case manager experience preferred. Qualified individuals will successfully complete the competency assessment.

Certification: OASIS Certification required. Home Health Coding Certification required. Must obtain within 12 months of hire.

License: Registered Nurse or Physical Therapist License in the State of Florida required. Valid Florida drivers license within 30 days of employment.
