Job summary

CenterWell Home Health seeks a licensed Physical Therapist Assistant to support patients in their homes under supervision of a Physical Therapist and Clinical Manager. You will help patients regain strength, mobility and independence with personalized rehab plans.

The role emphasizes teamwork, accurate documentation and adherence to infection control. A valid driver’s license and CPR certification are required for community-based services.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of changes in geriatric patients.

Responsibilities

• Administer PT treatment plan in patients' homes as developed by the Physical Therapist.
• Perform non-complex manual therapy and modalities.
• Document client records and submit documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Report equipment issues or staff changes to PT or Branch Director.
• Apply infection control and universal precautions in care activities.
