Qualifications

Responsibilities

• Provides direct physical therapy under the direction of a licensed physical therapist and per the physician’s plan of care.
• Delivers patient treatment to relieve pain and restore or improve function.
• Instructs patients in exercises, gait training, and use of assistive devices.
• Maintains and documents clinical notes and patient records within required timeframes.
• Collaborates with Home Care Team Members and participates in in-service education.
