Responsibilities

• Provides direct physical therapy under the direction of the licensed physical therapist.
• Treats patients to relieve pain, restore function, and maintain maximum performance.
• Directs and assists patients in active and passive therapeutic exercises, gait and functional training, and mobility transfers.
• Utilizes modalities such as heat, cold, water, light, electrical stimulation, ultrasound, and massage.
• Monitors patient response and reports changes to the physical therapist and organization staff.
• Instructs patients on use of wheelchairs, braces, crutches, canes, and prosthetic/orthotic devices.
• Maintains clinical records and notes within prescribed time frames; participates in team meetings and in-service education.

Provides direct physical therapy according to directions of physical therapistand in accordance with the physician's plan of care.2. Maintains necessary records; clinical notes and conference notes, which willbe incorporated into the patient’s clinical record in prescribed time frames.8. Participates in Organization Home Care Team Members meetings andOrganization in-service Education Program. **JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)*** A PT assistant must be licensed in State of Wisconsin* Must have graduated from a PT assistant program approved by CAPTE and have passed a national examination for PT assistants. Possesses and maintains current CPR certification.* The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
