Job summary

CenterWell Home Health is seeking a compassionate Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will deliver prescribed therapy, document progress, and coordinate care with a professional team to restore function and independence.

40 hours per week in a home health setting in the Las Vegas area. Requires an active Physical Therapy license, CPR certification, a valid driver's license, and the ability to work with geriatric

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of the geriatric population changes.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies and modalities.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment issues or staff changes.
• Apply infection control and universal precautions in patient care.
