Job summary

Beacon Home Health seeks a dedicated Physical Therapist Assistant to join our Home Health team serving Smithville and surrounding communities. You will provide treatment, educate patients, and support the plan of care under supervision of a PT, with travel to patient residences daily.

• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Instruct/assist patients in active and passive exercise, gait, ADLs, transfers, and safety.
• Observe and report patient responses to treatment to supervising PT and care team.
• Coordinate care with other health team members.

A few position highlights include:Provide services in accordance with laws governing the provision of physical therapy in home care, follow the treatment set only as defined by the supervising PT, perform only those procedures that he/she is qualified and trained to perform, and make all recommendations for changes in the plan of care to the supervising PT.Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.Observe, record, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
