Job summary

Beacon Home Health in Jefferson City, TN is seeking a dedicated Physical Therapist Assistant to join our home health team. You will provide therapy services and patient education under the supervision of a PT, travel to patient residences daily, and coordinate care with the healthcare team.

Required PTA license in-state, CPR certification, reliable transportation, and the ability to commute within the assigned geographic area.

• Must have reliable transportation to visit multiple patient residences daily.

Responsibilities

• Perform treatment and provide exercise instruction and patient education per plan of care.
• Instruct and aid patients in active and passive exercise, gait, ADL, transfer and safety training.
• Observe patient responses and report to supervising PT and care team; adjust plan as directed.
• Coordinate care with nursing, physicians and other team members.

A few position highlights include:Provide services in accordance with laws governing the provision of physical therapy in home care, follow the treatment set only as defined by the supervising PT, perform only those procedures that he/she is qualified and trained to perform, and make all recommendations for changes in the plan of care to the supervising PT.Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.Observe, record, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
