Job summary

VitalCaring is seeking a Physical Therapy Assistant (PTA) for Home Health, PRN. You will deliver one-on-one therapy in patients’ homes, helping them regain mobility and independence.

As part of a patient-centered team, you’ll follow plans of care under supervision, document progress in the EMR, educate caregivers, and collaborate with therapists, nurses, and care teams to ensure coordinated, compassionate care.

Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• 1+ year of home health experience preferred; CPR certified; valid driver’s license.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
