Alpine Care Home Health in San Antonio, TX is seeking a Physical Therapist Assistant per visit to deliver PT services, document progress, and support a patient-centered plan of care in the home setting.

You will supervise non‑professional home health aides, coordinate with PT, physicians and the care team, and help patients regain mobility while maintaining safety and independence.

A 40-pound lift, driving 2–4 hours daily, CPR certification, and strong communication are essential to success.

Qualifications

• Active CPR Certification is required.
• Valid driver’s license with a good driving record.
• Ability to lift 40 pounds from floor to shoulder.
• Ability to drive 2–4 hours per day.
• Excellent verbal and written communication skills.
• Proficiency with PC use including e-mail, clinical, word processing, and spreadsheets.
• Ability to assess and document patient needs and develop individualized care plans.

Responsabilités

• Provides physical therapy services as outlined in the patient Plan of Care under supervision.
• Schedules and adjusts visits and notifies the agency of changes.
• Reports significant findings to physicians, therapists, and supervisor.
• Submits visit notes and communication logs as required.
• Maintains open lines of communication with the care team.
