Job summary

Central Coast Home Health & Hospice is seeking a full-time or part-time Home Health Physical Therapist Assistant to serve the Southern San Luis Obispo area (Pismo Beach, Nipomo, Santa Maria). The role involves delivering high-quality in-home care, collaborating with a multidisciplinary team, and maintaining an independent, organized caseload with a flexible schedule.

Requirements include a California PTA license, CPR certification, valid driver’s license, reliable vehicle, and clean driving

Qualifications

• Current California PTA license is required.
• CPR certification required.
• Reliable vehicle to drive for work and a valid driver's license.
• Auto insurance coverage is required.
• Home health experience is a plus; new grads encouraged.

Responsibilities

• Provide home health physical therapy services to patients in their homes.
• Collaborate with an interdisciplinary team to address patient needs and ensure continuity of care.
• Manage own caseload and create a flexible work schedule.
