HealthPro Heritage is seeking a PRN Physical Therapist Assistant to deliver skilled services in patients' homes across the Chagrin Falls/Solon/Aurora/Northfield/Macedonia/Garfield Heights area. You will work under Physical Therapists, optimizing independence and function for clients in a home-based setting.

Qualifications

• Licensure: Valid state licensure as a Physical Therapist Assistant, or license eligible.
• Education: Associate’s degree in Physical Therapist Assisting from an accredited program.
• Collaboration: Strong communication and teamwork within a multidisciplinary care team.

Responsibilities

• Provide skilled Physical Therapy Assistant services in patients' homes according to treatment plans.
• Assist in implementing treatment plans with direct patient care, exercises, manual therapy, and modalities.
• Monitor and document patient progress and educate patients and families on protocols and self-care.
• Collaborate with Physical Therapists, other healthcare professionals and support staff to ensure cohesive care.
