Job summary

Interim HealthCare of Salem (Roanoke) is seeking a Home Health Physical Therapist Assistant (PTA) to provide one-on-one therapy in patients' homes throughout Roanoke and surrounding areas. The role supports independence, mobility, and daily functioning under the supervision of a Physical Therapist.

The PTA will travel within the service area, delivering personalized care, following a structured treatment plan, and coordinating with an interdisciplinary team to ensure continuity of care.

Qualifications

• Graduate of an accredited PTA program.
• Active Virginia PTA license.
• Minimum 1 year PTA experience; home health preferred.
• CPR certification and TB baseline screening.
• Strong communication and interpersonal skills.
• Good clinical judgment and ability to work independently.
• Reliable transportation and willingness to travel across the service area.

Responsibilities

• Provide home-based physical therapy under supervision.
• Assist patients with exercises, balance, gait, and mobility training.
• Monitor progress and report changes to the supervising PT.
• Reinforce and implement treatment plans.
• Educate patients and families on exercises and safety.
• Collaborate with RNs, LPNs, CNAs, OTs, and SLPs.
• Document visits and progress accurately.
• Maintain compliance with home health regulations.
