Job summary

OakMed Home Nursing & Rehabilitation is seeking licensed Physical Therapist Assistants (PTAs) for flexible, home-based opportunities in the Grand Rapids, MI area. You will provide one-on-one care in patients’ homes under the supervision of a licensed Physical Therapist, building a caseload that aligns with your availability and location.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Michigan PTA license (unrestricted).
• Valid driver's license and proof of automobile insurance.
• Reliable transportation to patient homes.
• Strong communication, organization, and time-management skills.
• Ability to work independently in the field under supervision of a licensed Physical Therapist.

Responsibilities

• Provide physical therapy treatment visits in patients' homes according to the supervising PT's plan of care.
• Assist patients with therapeutic exercises, mobility training, and functional activities.
• Help patients improve strength, mobility, balance, and independence.
• Educate patients and caregivers on prescribed exercises, mobility techniques, and safety practices.
• Monitor patient responses to treatment and communicate progress or concerns to the supervising PT.
• Complete accurate and timely visit documentation.
• Collaborate with the clinical team to support patient goals.
• Follow applicable clinical standards, documentation requirements, and HIPAA regulations.
