Job summary

CenterWell Home Health in Kentucky is hiring a Home Health Physical Therapist Assistant to support patients at home under the supervision of a Physical Therapist. You will administer the established treatment plan, monitor progress, and coordinate care with the clinical team to promote functional independence and safe mobility.

This role requires a current PT assistant license, valid driver's license and CPR certification, strong teamwork, and fluency in English.

Qualifications

• Current state license to practice Physical Therapy (PT).
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric physical, emotional, social changes.
• Read, write and speak fluent English.

Responsibilities

• Administer the physical therapy plan to patients, working onsite in their homes as developed by the PT.
• Administer non-complex active and passive exercises, manual therapy, modalities to patients.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
