Responsibilities

• Collaborate with case manager, primary therapist, and other healthcare professionals to plan discharge.
• Maintain productivity standards and adjust hours to patient needs.
• Participate in departmental meetings and performance improvement initiatives.
• Monitor patient response to treatment and discuss adjustments with the Physical Therapist.
• Educate patients, families, and caregivers on treatment plans and insurance guidelines.
• Document interventions and progress accurately and timely per standards.
• Other duties as assigned.
