Job summary

Creekside Village is seeking a Physical Therapist Assistant (PTA) to join our care team in a home-based setting. The PTA will provide rehabilitation services, monitor patient progress, and collaborate with supervising PTs to deliver high-quality care.

Ideal candidates will hold a valid PTA license, graduate from an accredited PT program, and maintain CPR certification. Preference is given to applicants with at least one year of home-health experience and strong patient-service skills to support
