Qualifications

• Current Physical Therapy licensure in state of Oregon.
• Current driver's license and vehicle insurance or access to dependable transportation.
• CPR certification (preferred) or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient's functional status and needs; assists therapy planning.
• Provides PT services within scope of practice and plan of care.
• Monitors patient responses and reports changes to supervisor/physician.
• Treats patients to relieve pain and restore function; documents progress.
• Educates patient and family about plan of care and progress toward goals.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and projected frequency of care.
