Home Health & Hospice Care is seeking a licensed Physical Therapist in New Hampshire to provide in-home rehabilitation in the Greater Nashua area. You will assess patients, develop individualized treatment plans, and help regain strength, mobility, and independence in the home setting.

The role requires 2+ years of hospital or clinical experience, CPR certification, and the ability to work autonomously with a patient-focused team.

Qualifications

• Licensed Physical Therapist in New Hampshire.
• Two or more years of hospital or clinic experience.
• CPR-certified with clean criminal and motor vehicle record.
• Master’s or Doctorate in PT preferred or homecare experience.

Responsabilités

• Develop individualized therapy plans to restore function and reduce pain.
• Evaluate patient progress and adjust plans based on measurable outcomes.
• Collaborate with physicians and homecare team to refine treatment plans.
• Complete patient admissions and initial assessments from medical records.
• Design and teach home exercise programs for patients and caregivers.
• Document goals and services in the electronic health record within timelines.
• Travel within the agency’s territory to provide in-home therapy as needed.
• Supervise PTAs in accordance with scope of practice.
