Job summary

CenterWell Home Health seeks a Home Health Physical Therapist to plan and administer PT in patients' homes, aiming to restore function and independence. You will assess progress, tailor rehab programs, and guide care teams with clear instructions.

This role emphasizes timely documentation and reporting in a patient-centered setting. Experience collaborating with healthcare professionals is required; a PT license and CPR certification are essential.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed PT treatment in patients' homes to attain highest physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes and discharge summaries.
