Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and mobility in their homes. You will report to the Clinical Manager and deliver personalized care focused on functional improvement and independence.

The role includes planning and administering therapy, assessing progress, coordinating with the care team, and documenting outcomes. A current PT license and CPR certification are required, with outpatient/home-based experience preferred.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatments for patients, working onsite in their homes.
• Assess and document patient's physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with the physician's plan and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family; submit all required evaluations and discharge summaries.
