Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their own homes. Reporting to the Clinical Manager, you will deliver personalized therapy aimed at functional improvement and safer daily living.

You will assess patients, plan rehabilitation programs, supervise PTAs and Home Health Aides, document progress, and communicate with physicians and families to support optimal outcomes. This is a full-time, in-home role.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes.
• Assess and document patients' physical strengths.
• Develop and implement rehabilitation programs per physician's Plan of Treatment and goals.
