Job summary

CenterWell Home Health is seeking a Physical Therapist to join the care team. You will report to the Clinical Manager and help patients regain strength, mobility, and independence through personalized home-based therapy.

Responsibilities include planning and delivering PT, assessing patient progress, developing rehabilitation programs, supervising PTAs and HHAs, documenting interventions, and communicating updates to the care team and family. TB screening and driver requirements apply.

Qualifications

• Degree from an accredited Physical Therapy Program (APTA approved).
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop rehabilitation programs consistent with the physician’s Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager, care team, patient, and family; submit evaluations and discharge summaries.
