Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement a rehab program aligned with the Plan of Treatment.
• Provide clear care instructions to PTAs and Home Health Aides and oversee their work.
• Document observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient and family; submit required documents.
