Job summary

Millennium Home Care is seeking a PRN Physical Therapist to provide in-home patient evaluations, treatments, and education in Florida. You will conduct pain-relief and mobility-enhancing therapies, guide patients in home exercise programs, and collaborate with a multi-disciplinary team to ensure quality outcomes.

The role requires an active Florida PT license, at least 1 year of clinical experience, and reliable transportation to visit patients at home.

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health setting.
• Reliable transportation to visit patients at their homes.

Responsibilities

• Assess patients' physical abilities and develop individualized treatment plans.
• Apply therapeutic techniques including manual therapy, exercises, and gait training.
• Help patients develop compensatory strategies and training with ADLs.
• Educate patients and families on home exercise programs and injury prevention.
• Collaborate with physicians, nurses and other therapists for comprehensive care.
• Maintain accurate documentation and compliant records.
