Pinnacle Home Care is seeking PRN Physical Therapists to deliver home-based PT services across Florida. You will perform initial and ongoing patient assessments, develop individualized treatment plans, and collaborate with physicians and an interdisciplinary team to optimize outcomes.

Qualificações

• Must have current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health preferred).
• Excellent communication, organizational, and time management skills.
• Must have a valid driver’s license and access to a reliable vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Sufficient hearing, eyesight, and dexterity for assessments and care techniques.

Responsabilidades

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families/caregivers on therapy programs.
• Maintain accurate clinical records and report changes to the physician.
• Supervise Physical Therapy Assistants in accordance with policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care.
• Identify patient and family/caregiver needs for additional services and referrals.
• Confirm daily visit schedules for seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.

Key Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.

• Confirm daily visit schedules to ensure seamless coordination with other staff.


• Communicate patient progress and changes with physicians and care teams.


• Document all visits and progress notes on the same day of service.

• Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.


• Proficiency with electronic medical records is a plus.
