Job summary

Beacon Home Health, an affiliate of The Pennant Group, is seeking a licensed Physical Therapist to join our Home Health team serving Cleveland, TN and surrounding communities. You will evaluate patients, develop and implement a plan of care, and monitor progress at patient residences on the road.

Responsibilities include assessing functional status, guiding therapists and caregivers, and ensuring treatment aligns with physician directives.

Qualifications

• Current PT license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Evaluate the patient's functional status and physical therapy needs.
• Assist the physician in the development of the therapy plan of care.
• Observe and report the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician.
• Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
• Instruct and inform participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
