Allegiance Home Health is seeking a dedicated Home Health Physical Therapist to deliver skilled therapy in patients' homes, with a per-diem start that will transition to full-time. Requires at least one year of home health experience, a PT license, and strong communication and organization.

Valid driver’s license and reliable transportation are required. The role emphasizes patient-centered care, collaboration with interdisciplinary teams, and ongoing evaluation of progress in home settings.

Qualifications

• One year of home health experience is required.
• Current state licensure as a Physical Therapist.
• Master's or Doctoral degree in Physical Therapy from an accredited program.

Responsibilities

• Conduct thorough assessments and evaluations of patients' physical abilities and needs in a home setting.
• Develop and implement individualized treatment plans based on patient assessments and clinical findings.
• Utilize various therapeutic techniques, exercises, and modalities to improve patients' mobility, strength and overall function.
• Educate patients and their families on rehabilitation techniques, safety precautions, and home exercise programs.
• Monitor and document patient progress, adjusting treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
