Qualifications

• Active SC Physical Therapist license required.
• Current American Heart Association BLS certification required.
• Driver's license and automobile insurance as required by law.

Responsibilities

• Provide physical therapy services per physician orders and treatment plans.
• Assess patient needs and design individualized therapy programs.
• Educate patients and caregivers on goals and home exercise programs.
• Collaborate with physicians, nurses, case managers and other health‑care professionals for coordinated care.
• Maintain accurate and timely clinical documentation.
• Participate in performance improvement initiatives, competency training and professional development.
• Serve as a clinical resource and mentor for students, volunteers and staff as appropriate.
• Ensure compliance with state, federal, and accreditation standards for home health services.
