Job summary

Signature Healthcare at Home is hiring a full-time Physical Therapist to serve the Westside Portland Metropolitan Area, including Beaverton and Tigard, with a field-based caseload across SW/NW regions. You will evaluate patients at home, develop mobility-focused plans, and provide gait training, balance work, and home exercise programs.

The role emphasizes independent practice, strong communication, and collaborative care with an interdisciplinary team.

Qualifications

• Oregon PT license required or ability to obtain
• 1 year of PT experience preferred; home health a plus
• Strong communication and clinical reasoning skills

Responsibilities

• Conduct home-based physical therapy evaluations.
• Develop individualized treatment plans focused on functional mobility and independence.
• Deliver therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
• Perform home-safety assessments and recommend appropriate DME.
• Manage a field-based caseload with efficiency and autonomy.
• Collaborate with interdisciplinary team members to ensure coordinated care.
• Maintain accurate documentation in accordance with regulatory standards.
• Support safe transitions from hospitals, SNFs, and outpatient settings.
