Job summary

Pinnacle Home Care in Florida is seeking a Physical Therapist (PT) for full-time, offering compassionate home health care across Highlands and South Polk counties. You will assess patients, create and adjust treatment plans in collaboration with physicians, and educate families throughout the care journey.

The role requires a current PT license, a degree in physical therapy, and at least one year of PT experience (home health preferred).

Qualifications

• Current PT license in the state.
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health preferred).
• Excellent communication, organizational, and time management skills.
• Valid driver’s license and access to a reliable vehicle.
• Ability to visit homes that may not be wheelchair accessible.

Responsibilities

• Perform initial and ongoing patient assessments and documentation.
• Develop and implement tailored treatment plans with physicians and clinicians.
• Collaborate with the care team to optimize patient outcomes.
• Provide education to patients and families on therapy programs.
• Maintain accurate clinical records and report changes to physicians.
• Supervise Physical Therapy Assistants according to policy.
• Identify needs for additional services and refer appropriately.
• Coordinate daily visit schedules with staff.
• Document all visits and progress notes the same day of service.
