Qualifications

• Current Physical Therapist license in the state.
• Minimum of one year of recent PT work experience.
• Graduation from an APTA-approved PT program or AMA/AHEA-equivalent.
• Strong computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Develop and implement individualized care plans with manual therapy, gait training, balance and other interventions.
• Continually assess and revise PT care plans.
