Job summary

Ascension at Home together with Compassus is seeking a Home Health Physical Therapist to deliver in-home physical therapy services to patients in Wisconsin. Qualifications

• Bachelor of Science in Physical Therapy required.
• Current State License as a Physical Therapist required.
• CPR certification required; TB screen and Hepatitis consent/declination as applicable.
• Two years’ experience as a Physical Therapist in acute care or rehabilitation setting preferred.

Responsibilities

• Provide in-home physical therapy to patients in their homes.
• Assess, plan, and implement individualized treatment plans while engaging family/caregivers.
• Collaborate with care teams to ensure quality patient outcomes.

Two years’ PT experience
