Job summary

Nightingale Home Healthcare is seeking a part-time Physical Therapist (PT) to provide skilled therapy in patients' homes across Chicago and surrounding areas. You will evaluate, plan, and deliver therapy, supervise PTAs and HHAs, and collaborate with physicians to ensure high-quality care.

Ideal candidates have an active PT license, at least 1 year of home health experience, and are comfortable with WellSky EMR. Flexible scheduling and strong team support are offered.

Qualifications

• Active Physical Therapist license.
• Minimum 1 year PT experience, home health preferred.
• EMR experience with WellSky (Kinnser) is a plus.
• Current TB test and CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients within 72 hours of referral.
• Develop and implement individualized Plans of Care.
• Provide therapy and supervise PTAs and HHAs.
• Communicate with physicians and interdisciplinary team.
• Maintain accurate HIPAA-compliant documentation in EMR.
• Assess treatment effectiveness and update care plans.
