Job summary

Nightingale Home Healthcare in Chicago and surrounding areas is seeking a dedicated Physical Therapist to join our home health team on a full-time basis.

You will evaluate patients within 72 hours of referrals, develop individualized Plans of Care, provide therapy services, and supervise PTAs and HHAs while coordinating with physicians and the care team. Documentation must be accurate and HIPAA compliant.

Qualifications

• Active Physical Therapist license required.
• Minimum 1 year PT experience; home health/OASIS preferred.
• WellSky (Kinnser) EMR experience is a plus.
• Current TB test within 12 months.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients within 72 hours of physician referral.
• Develop and implement individualized Plans of Care.
• Provide therapy services and supervise PTAs and HHAs.
• Communicate with physicians and interdisciplinary team.
• Maintain accurate, timely documentation in EMR.
