Job summary

Patriot Homecare is seeking a dedicated Physical Therapist to provide patient-centered home health services in Trumbull County, Ohio. You will assess, plan, and implement therapy programs, coordinating with physicians and an interdisciplinary team to optimize recovery and mobility for patients in their homes.

The role requires an active Physical Therapy license, at least one year of home health experience, and CPR (BLS).

Qualifications

• Must have active Physical Therapy license in the state
• Minimum of 1 year of institutional/home health experience
• Must have a current CPR (BLS) certification
• Criminal background check required
• Ability to drive and visit patients in their homes
• Excellent communication and teamwork skills

Responsibilities

• Provide initial and ongoing comprehensive assessments of the patient's needs, including OASIS assessments
• Establish a treatment plan in collaboration with the physician and revise as necessary
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy
• Assist the physician evaluating patients by applying diagnostic and prognostic tests
• Teach, supervise, and counsel the family and patient in the physical therapy program
• Write clinical/progress notes on the date of the visit and integrate into patient records within 72 hours
