Job summary

Trilogy Home Healthcare Ocala is hiring a Physical Therapist to provide one-on-one PT services in patients' homes in Lake County, FL. You will help improve mobility, strength, and balance while setting goals and promoting independence. A valid Florida PT license and a driver’s license are required, with home health experience preferred.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of experience required, Home Health experience preferred.
• Sound judgement and a passion for patient care and outcomes.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Help patients improve mobility, strength, and balance.
• Set and manage care goals that promote recovery and independence.
• Evaluate the home environment for safety and mobility needs.
