Qualifications

• Current California Physical Therapist license is required.

Responsibilities

• Evaluate and treat patients in patients' homes to improve mobility and independence.
• Develop and implement treatment plans and document visits.
• Educate patients and caregivers about care and safety.
• Coordinate with referral network and scheduling.
