Job summary

Conemaugh Home Health, part of the LHC Group and a member of the Optum family, seeks a Physical Therapist for home health. You will assess patient needs, determine a treatment plan, and implement interventions to improve wellbeing and function, with collaboration across the health care team.

Responsibilities include evaluating functional status, developing therapy plans, documenting progress, and communicating with patients families.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs according to plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient progress to the supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients and families about plan of care, progress, and limitations.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
