Job summary

Trilogy Home Healthcare, a Humana company, is seeking a Physical Therapist to perform home health visits in Palm Coast, FL. The role offers PRN/per diem scheduling with meaningful patient impact and opportunities to advance in a stable, team-driven environment.

You will evaluate home safety, improve mobility and balance, and set recovery-focused care goals for patients in their homes, with autonomy and support from leadership.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license with active vehicle insurance and clear MVR.
• 1+ year of PT experience; home health experience preferred.
• Sound judgement and a passion for patient care.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Help patients improve mobility, strength, and balance.
• Set and manage care goals that promote recovery and independence.
• Evaluate the home environment for safety and mobility needs.
