Job summary

Amedisys is seeking a licensed Physical Therapist to serve patients in the Bellevue, WA area. You will help patients maintain, restore and improve movement and function in the home setting, with regular reexaminations and discharge planning as needed.

Qualifications

• Must hold a current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• At least 1 year of home health care experience or confirmation of mentoring if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations at least every 30 days, adjust plans as needed, and perform discharge planning.
• Perform other duties as assigned.
