CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home physical therapy, helping patients regain strength and independence. You will assess, plan, and implement rehab programs, document progress, and coordinate with a care team to ensure comprehensive patient care.

• Current and unrestricted Physical Therapy license.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist.
• Valid state driver's license and personal vehicle liability insurance.

Responsabilidades

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to support physician evaluation.
• Develop and implement a rehabilitation program aligned with physician plans and patient goals.
• Provide PTAs and Home Health Aides with clear care instructions and oversee documentation.
• Document patient observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient, and family; submit evaluations and discharge summaries.
